RN Care Coordinator - Group Home

About the Role

Provide skilled nursing services and home-based care to individuals with medical needs in group homes across multiple New Jersey locations. Coordinate care among families, staff, physicians, and hospitals to ensure patient goals are met.

Responsibilities

• Assess patients, document progress, and report changes to physicians.
• Educate caregivers and direct support professionals on care plans, medications, therapy, diet, and safety.
• Coordinate care among families, staff, physicians, and hospitals to align with patient care goals.

Requirements

• Graduate of an accredited nursing program with an active RN license in New Jersey.
• Minimum of 2 years of nursing experience, preferably with developmentally challenged individuals or in a group home setting.
• CPR certification (must be obtained within 60 days of hire).
• Knowledge of home health and group home regulations.
