About the Role
Provide skilled nursing services and home-based care to individuals with medical needs in group homes across multiple New Jersey locations. Coordinate care among families, staff, physicians, and hospitals to ensure patient goals are met.
Responsibilities
- Assess patients, document progress, and report changes to physicians.
- Educate caregivers and direct support professionals on care plans, medications, therapy, diet, and safety.
- Coordinate care among families, staff, physicians, and hospitals to align with patient care goals.
Requirements
- Graduate of an accredited nursing program with an active RN license in New Jersey.
- Minimum of 2 years of nursing experience, preferably with developmentally challenged individuals or in a group home setting.
- CPR certification (must be obtained within 60 days of hire).
- Knowledge of home health and group home regulations.
Skills
- Clinical proficiency in home-based and group home care.
- Strong communication and compassion.
