RN Navigator - Pediatric Sleep & Neurology Clinics

About the role

The Sanford Health Pediatric Sleep & Neurology Clinic is seeking an RN Navigator to support the coordination of care for patients and families.

Responsibilities

• Coordinate care to ensure patients and families have the necessary treatment, education, and resources.
• Collaborate with healthcare team and other professionals to ensure timely sequencing of care.
• Support patients and families by removing barriers to care.
• Evaluate patient responses using structured care methodologies.
• Work closely with ancillary and support services.
• Solve problems and implement process improvements when needed.
• Facilitate multidisciplinary teams and implement best practices.

Requirements

• Bachelor's degree in Nursing preferred in Bemidji and Rapid City.
• Bachelor's degree in Nursing required in Fargo, Sioux Falls, Bismarck, or enrolled in a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position.
• Current RN license with the State Board of Nursing.
• Required to obtain and maintain department-specific competencies and certifications.

Qualifications

• Minimum of two years' clinical experience preferred.
• Strong communication skills.
• Self-motivated and able to work independently.

Skills

• Knowledge of and utilizes appropriate age-related patient care protocols.
• Ability to integrate health care across the continuum of care.
• Interdisciplinary communication and early intervention skills.
