Job Summary
The Perinatal Nurse Navigator at HCA HealthONE Presbyterian St. Luke's serves as a member of the multidisciplinary team and advocates for patients with complex medical conditions. Key responsibilities include:
- Serves as a patient advocate
- Affords assessments of medical and psychosocial needs related to primary medical or surgical conditions
- Identifies patient needs and barriers to care and facilitates access to potential resolutions
- Assists in removing health disparities and provides appropriate education, outreach, and support to patients and families
- Ensures patients are empowered to manage their healthcare needs
- Guides patients and families through the healthcare system by assisting with access issues and streamlining the intervention process
- Coordinates the defined plan of care, assists with appointments, resolves barriers, and connects with community resources
- Manages resources to achieve efficient, high-quality outcomes for patients
- Collects and tracks data to promote the navigation program's performance
Qualifications
- 1-3 years of experience in Nursing
- 3+ years of experience as a Nurse Navigator and/or patient educator in service line
- Associate’s Degree in Nursing required
- Bachelor’s Degree in Nursing preferred
- Basic Cardiac Life Support (preferred if remote position)
- Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation.

What You Will Do
In this role, you will:
- Serve as a patient advocate

What You Will Need
