About the role
Under general supervision and in accordance with Bronson’s policies, procedures, and guidelines, the registered nurse (RN) or licensed practical nurse (LPN) manages care delivery for patients throughout their length of stay to achieve quality outcomes and timely discharge by:
- Providing selected aspects of physical care within the scope of professional nursing practice, appropriate to the resident’s acuity and complexity.
- Utilizing the nursing process: assessment, planning, diagnosing, implementation, and evaluation.
- Coordinating care with other interdisciplinary team members.
- Delegating tasks within the scope of professional nursing practice and the abilities of LPNs and CNAs.
- Documenting and communicating the resident’s response to care, resource consumption, and anticipated needs.
Employees providing direct patient care must demonstrate competencies specific to the population served.
Responsibilities
- Collects, assesses, and documents comprehensive data pertinent to the patients’ health or situation and implements appropriate interventions.
- Identifies expected outcomes for an individualized care plan, incorporating patient/family input, values, ethical, and cultural considerations.
- Develops individualized interventions with input from patients, families, and staff when appropriate.
- Administers prescribed medications and treatments in accordance with approved nursing techniques and policies.
- Maintains awareness of patient comfort and safety needs.
- Initiates and coordinates a patient education plan based on individualized needs.
- Manages the care plan by organizing and delegating patient care interventions.
- Evaluates the effectiveness of care provided by self and others.
- Rotates to all units within the facility based on need.
- Completes all required education, attends staff meetings, skills fairs, and computer-based learning as directed.
- Accepts responsibility for on-call expectations.
Requirements
- Associate’s degree, Diploma, or Bachelor’s degree in Nursing required.
- Licensed RN or LPN in good standing with the State of Michigan.
- BLS certification required by completion of core orientation.
- Successful completion of Bronson Commons’ clinical orientation program.
- VAST training, as provided by Bronson Commons.
- Experience in Long Term Care, Rehab, or general Geriatrics preferred.
- Geriatric Certification preferred.
- Basic computer skills required.
- Ability to read, interpret, and analyze data from various computer systems and equipment to set appropriate plans of action.
- Must be able to constantly communicate verbally, in writing, and electronically.
Skills
- Work produces high levels of mental/visual fatigue, including interactive, repetitive, or detailed tasks requiring sustained alertness and concentration (e.g., operating a computer or CRT 40–70% of the time).
- Regularly lifts bulky or moderately heavy weights (up to 50 pounds) and occasionally assists with heavier tasks or equivalent effort in pushing, pulling, or handling material, equipment, or other objects.
Schedule
- Second Shift (Part time, 24 scheduled weekly hours).
- 48 hours per pay period.
