LPN/RN part-time 6pm-12am

About the role

The LPN is responsible for providing quality patient-centered, evidenced-based care to our residents. Duties include initial data collection and participating in the implementation of person-driven care plans.

Responsibilities

• Utilize principles of Relationship Based Care
• Perform duties as assigned
• Initial data collection on all residents
• Participate in implementing person-driven care plans

Requirements

• Current state licensure as a Practical Nurse in good standing
• Must be able to read, write, and communicate in English
• Clinical experience preferred

Qualifications

• Dynamic and compassionate
• Experience in senior care preferred

Skills

• Strong communication skills
• Ability to work independently and as part of a team
