About the role

The RN Long Term Service Program Coordinator is responsible for overseeing the coordination and management of long-term care services in the Concord area.

Responsibilities

  • Oversee the implementation and coordination of long-term care programs
  • Manage staff and ensure compliance with regulatory standards
  • Develop and maintain relationships with healthcare providers and community organizations
  • Conduct regular assessments of service quality and make necessary improvements
  • Collaborate with other departments to ensure seamless service delivery

Requirements

  • Bachelor’s degree in Nursing or related field
  • Current RN license in good standing
  • Minimum 3 years of experience in long-term care management
  • Strong organizational and leadership skills
  • Excellent communication and interpersonal skills

Qualifications

  • Experience working in a healthcare setting
  • Knowledge of long-term care regulations and best practices
  • Ability to manage multiple tasks and priorities
  • Proficiency in Microsoft Office Suite
