About the role
The RN Long Term Service Program Coordinator is responsible for overseeing the coordination and management of long-term care services in the Concord area.
Responsibilities
- Oversee the implementation and coordination of long-term care programs
- Manage staff and ensure compliance with regulatory standards
- Develop and maintain relationships with healthcare providers and community organizations
- Conduct regular assessments of service quality and make necessary improvements
- Collaborate with other departments to ensure seamless service delivery
Requirements
- Bachelor’s degree in Nursing or related field
- Current RN license in good standing
- Minimum 3 years of experience in long-term care management
- Strong organizational and leadership skills
- Excellent communication and interpersonal skills
Qualifications
- Experience working in a healthcare setting
- Knowledge of long-term care regulations and best practices
- Ability to manage multiple tasks and priorities
- Proficiency in Microsoft Office Suite
