MDS Coordinator RN - Long Term Care

About the role

Nazareth Home is seeking an experienced and detail-oriented MDS Coordinator-RN (Registered Nurse) to join their RAI/MDS program in a skilled nursing environment. This position is based at their Clifton campus located at 2120 Payne St.

Responsibilities

• Aid in implementing the RAI/MDS process with federal regulations.
• Participate in care planning and work closely with the nursing department for quality care.
• Oversee the coordination and participation in the completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations.
• Ensure timely and accurate completion of assessments (OBRA, PPS, Managed Care).
• Carefully coordinate with nursing, therapy, and billing teams to support resident care and reimbursement accuracy.
• Participate in the utilization review, case mix management, and Triple Check processes.
• Ensure compliance with federal and state regulations, including MDS submission requirements.
• Develop and oversee individualized care plans in collaboration with the interdisciplinary team.
• Support audits, surveys, and Medicare/insurance reviews.
• Provide leadership and guidance to the MDS team and nursing staff.

Requirements

• Graduate from an accredited school of Nursing.
• Licensed by the Kentucky Board of Nursing.
• Current CPR certification.
• One year minimum of experience as a charge nurse in a Long-Term Care setting, preferred.
