About the role
Avera is seeking a RN-MDS Coordinator to join our team at Eureka Health Care Center. This position offers varied schedules and is responsible for daily coordination of resident care, overseeing MDS assessments, and maintaining compliance with regulatory requirements.
Responsibilities
- Audits resident charts, care plans, and charting to ensure documentation supports the care provided and reimbursement level.
- Completes and transmits MDS assessment and reports in accordance with regulatory requirements and facility policies and procedures.
- Identifies and communicates need for significant change in status assessments.
- Maintains a viable working relationship with all disciplines involved in the planning of resident care.
- Schedules MDS assessments per guidelines, as assigned and maintains a monthly calendar.
- Notifies members of the IDT of changes or additions to monthly Care Plan Meeting schedule, for new admission, change of condition or interim meetings.
- Electronically submits MDS information. Reviews, revises, corrects, and resubmits as appropriate.
- Contributes to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care.
- Participates in facility QA initiatives.
Requirements
- The individual must be able to work the hours specified.
- Must have an active license in the state of practice.
- Experience in Long Term Care nursing with MDS experience is preferred.
- Experience in Geriatric or rehabilitation nursing is preferred.
Qualifications
- Registered Nurse (RN) - Board of Nursing.

- Compliance with safety principles, laws, regulations, and standards associated with, but not limited to, CMS, The Joint Commission, DHHS, and OSHA if applicable.
Skills
- Strong organizational and communication skills.
- Knowledge of MDS assessment and reporting processes.
- Ability to maintain confidentiality and work effectively in a team environment.

The pay range for this position is listed below. Actual pay rate dependent upon experience. $31.00 - $46.25
