MDS Coordinator (RN)

About the role

We are currently searching for a MDS Coordinator to join our friendly, caring and supportive team at Avantara Palos Heights. Our facility is rapidly growing and we are committed to investing in our team members by providing opportunities for career advancement and the tools needed to succeed.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan for their individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Ensure residents are comfortable and safe by utilizing knowledge, skills, and attention to detail.

Requirements

• Minimum of an unencumbered State of Illinois R.N. License.
• C.P.R. Certification.
• As a minimum, a graduate of an accredited nursing program.
• Prior experience as a MDS Coordinator is preferable but not required.
• One (1) year of experience as an R.N. in a Skilled Nursing Facility setting.
• One (1) year of experience as a C.N.A.

• Excellent communication and interpersonal skills.
• Strong organizational and time management abilities.
• Ability to work independently and as part of a team.
• Knowledge of Skilled Nursing Facility operations and regulations.
