Key Responsibilities
- Completing and submitting accurate MDS assessments in accordance with federal, state, and facility guidelines.
- Collaborating with nursing staff, therapists, and other healthcare professionals to gather information and complete comprehensive resident assessments.
- Developing individualized care plans based on MDS assessments and resident goals, preferences, and needs.
- Monitoring and updating resident care plans as needed, ensuring documentation reflects changes in condition and treatment.
- Coordinating with billing and reimbursement staff to ensure accurate coding and documentation for Medicare, Medicaid, and private insurance claims.
- Participating in interdisciplinary team meetings, care conferences, and quality improvement initiatives to enhance resident care and outcomes.
- Providing education and training to staff on MDS assessment processes, care planning, and regulatory requirements.

Qualifications
Requirements include:
- A current New York State RN license.
- Experience in MDS assessment and care planning.
- Strong organizational and communication skills.
- Ability to work independently and collaboratively with interdisciplinary teams.
Skills

Pay
The salary range for this position is $50,000 - $60,000 annually, commensurate with experience.
