RN Hospice Case Manager

About the role

Empath Trustbridge Hospice is seeking a RN Hospice Case Manager to join their team in Broward and Southern Palm Beach County. The position involves working Monday - Friday from 8:30am - 5:00pm with a weekend requirement.

Responsibilities

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the Interdisciplinary Group (IDG)
• Use clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

Requirements

• Registered Nurse currently licensed in the State of Florida
• Current Basic Life Support Certification
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus

Qualifications

• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license

• Strong communication and interpersonal skills
• Ability to manage multiple tasks and prioritize effectively
• Knowledge of hospice care principles and practices
