About the role

This position is based in Sebring, FL and requires daily travel within Highlands and/or Hardee County. It is full-time, Monday through Friday 8 am – 5 pm with participation in the on-call rotation.

Salary

Salary Range: $80,000–$85,000 annually, with additional bonus opportunities. Final compensation may vary based on location, job-related knowledge, skills, and experience.

Role of the RN Case Manager

As a Hospice Case Manager, you will provide, coordinate, and direct hospice nursing and palliative care to terminally ill patients in their home or wherever they call home. You will closely collaborate with the attending physician, patients and their families, and other members of the care team to achieve excellent pain and symptom management and high-quality end-of-life care for adult patients.

Nuts & Bolts of the Job

  • Work Monday–Friday, 8 am–5 pm with a potential weekday on-call and holiday coverage schedule
  • Case manage 15–18 patients
  • Attend Interdisciplinary Team Meetings and contribute to team huddle calls
  • Receive daily patient updates
  • Provide case management including assigning aide visits, coordinating care with social workers, chaplains, volunteers, and the bereavement team
  • Develop and nurture relationships with facility staff and community leaders

Orientation, Onboarding & Training

Orientation for an RN averages 3–4 weeks and is individualized to your needs and experience.

Responsibilities

  • Perform patient assessments and collaborate with the care team to develop and implement a plan of care
  • Make referrals to other hospice care disciplines as needed or document rationale for not doing so
  • Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness according to care center policies
  • Record pain/symptom management changes/outcomes as appropriate
  • Teach patients, families, and caregivers appropriate patient care techniques
  • Regularly communicate with clinical manager and care team
  • Perform on-call responsibilities and on-call services to patients/families as assigned
  • Other duties as assigned

Qualifications

  • Current RN license in the state(s) where you will work
  • Minimum 1 year of clinical RN experience in an acute healthcare setting
  • Current CPR certification

Preferred:

  • Hospice or home health experience
  • Strong communication and interpersonal skills
  • Ability to work independently and manage multiple priorities
  • Comfortable with daily travel between facilities
  • Proficient in electronic medical record (EMR) systems
