RN Hospice Case Manager
About the role
Award-winning Hospice Trustbridge Hospice, a proud member of Empath Health, seeks a dedicated Hospice Registered Nurse (RN) Case Manager to join our hospice team in the Boynton Beach/Delray Beach area. The ideal candidate will work a flexible schedule of Monday - Friday 8:30am - 5:00pm with a weekend requirement.
Responsibilities
- Coordinate comprehensive care for patients and families in collaboration with the primary physician and interdisciplinary care team
- Identify and assess patient and family needs, ensuring individualized and responsive care
- Serve as an integral member of the Interdisciplinary Group (IDG), contributing to and following a personalized plan of care
- Utilize strong clinical assessment, monitoring, intervention, and teaching skills to maximize patient comfort and quality of life
- Provide compassionate guidance and education to patients and caregivers throughout the hospice journey
Requirements
- Registered Nurse currently licensed in the State of Florida
- Current Basic Life Support Certification
- One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
- Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus
Qualifications
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license
