About the role
This position is based in Sebring, FL and requires daily travel within Highlands and/or Hardee County. It is full-time, Monday through Friday 8 am – 5 pm with participation in the on-call rotation.

Salary
Salary Range: $80,000–$85,000 annually, with additional bonus opportunities. Final compensation may vary based on location, job-related knowledge, skills, and experience.

Role of the RN Case Manager
As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families.
Nuts & Bolts of the Job
- Hospice RN Case Managers work Monday – Friday, 8 am – 5 pm with a potential weekday on-call and Holiday coverage schedule
- Case Manage 15–18 patients
- Attend Interdisciplinary Team Meetings and contribute to team huddle calls; receive updates on your patients daily
- Provide Case Management Assign Aide Visits
- Coordinate Care with Social Workers, Chaplains, Volunteers and Bereavement team
- Develop and nurture relationships with facility staff and leaders throughout the community

Orientation, Onboarding & Training
Orientation averages 3–4 weeks for an RN, individualized according to your needs and experience.
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
- Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies
- Records pain/symptom management changes/outcomes as appropriate
- Teaches patients, families, and caregivers appropriate patient care techniques
- Regularly communicates with clinical manager and care team
- Performs on-call responsibilities and on-call services to patients/families as assigned
- Other duties as assigned
Qualifications
- Current RN license in the state(s) where you will work
- Minimum 1 year of clinical RN experience in an acute healthcare setting
- Current CPR certification
- Preferred: Hospice or home health experience
- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
