RN Hospice Admission - Full Time (In Field)

About the role

The Hospice Admission RN is a licensed nurse responsible for educating patients and families about hospice services, assessing eligibility, and facilitating admission processes.

Responsibilities

• Conduct head-to-toe assessments
• Review medical histories and medications
• Develop individualized care plans
• Obtain physician orders
• Coordinate resources for safe, comfortable home or facility care
• Provide emotional support
• Discuss advance directives
• Collaborate with interdisciplinary teams to ensure comprehensive patient care

Qualifications

• Associate's or bachelor's degree in nursing
• Active RN license (single or multi-state)
• BLS certification
• Valid driver's license with private vehicle insurance
• Minimum two years of acute care experience (hospice experience preferred)
• Strong empathy, compassion, consultative skills, and commitment to patient-centered care
• Compassionate, attentive, collaborative, and dedicated to enhancing patient and family experience in hospice care
• Adherence to professional practice standards, ethical guidelines, and organizational policies
