Hospice Field Admissions RN - Full-Time

About the role

Conduct initial physical and psychosocial assessments of potential hospice patients in various settings such as private homes, hospitals, or nursing facilities.

Responsibilities

• Develop individualized care plans and set goals based on patient needs
• Make recommendations to ensure admission aligns with NHPCO standards

Requirements

• Associate’s or Diploma in Nursing (BSN preferred)
• At least 2 years of experience in med/surg, community health, or oncology
• Active NYS RN license
• Valid driver’s license
