About the role
Perform initial physical and psychosocial assessments of potential hospice patients in various settings such as private homes, hospitals, or nursing facilities. Develop individualized care plans, set goals, and make patient-specific recommendations while adhering to standards established by the NHPCO.
Requirements
- Associate’s or Diploma in Nursing (BSN preferred)
- Current NYS RN license
- Valid NYS driver’s license
Qualifications
Prior experience in med/surg, community health, or oncology nursing is preferred.
