Hospice Admissions RN

About the role

The Hospice Admissions Nurse facilitates smooth patient admissions into hospice care, providing compassionate nursing support and ensuring accurate documentation.

Responsibilities

• Conduct comprehensive assessments of new patients, including medical history and current health status.
• Collaborate with interdisciplinary teams to develop personalized care plans.
• Communicate effectively with patients, families, and caregivers about the admission process and services.
• Complete and ensure proper documentation, including consent and insurance forms.
• Coordinate with social workers, chaplains, and volunteers for necessary assessments and services.
• Administer medications and treatments per physician orders, emphasizing pain and symptom management.
• Monitor patients' vital signs and overall condition during admission, reporting changes promptly.
• Provide emotional support and reassurance to patients and families during admission.
• Ensure compliance with regulatory standards and participate in team meetings.
• Perform additional duties assigned by supervisory staff.

Qualifications & Skills

• Valid RN license, with at least 2 years of RN experience.
• Strong assessment, critical thinking, and clinical judgment skills.
• Excellent communication and interpersonal abilities.
• Knowledge of hospice regulations and documentation standards.
• Organizational and time-management proficiency.
• Valid driver’s license and reliable transportation for home visits.

Physical & Emotional Requirements

• Ability to work in various medical environments, perform physical tasks such as lifting, transferring, and assisting with mobility.
• Stamina for extended periods of standing, walking, and performing physically demanding tasks.
• Emotional resilience to handle complex healthcare needs and high-stress situations.
• Capability to use fine motor skills for precise movements and steady hands for documentation.
