About the role

The Registered Nurse (RN) will provide care and services for a group of patients, visiting them where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psycho-social assessments, interventions, and knowledge of community resources. The RN will work in either a Case Management, Admissions, or Resource role.

Responsibilities

  • Work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Work closely with patients on a 1-1 level while receiving strong support from a patient care team.
  • Operate independently on a day-to-day basis with activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  • Manage time effectively to handle visits, documentation, meetings (e.g., scheduled visits, required meetings), and respond to urgent needs by prioritizing and triaging multiple concerns.
  • Learn and use an electronic medical records system.
  • Drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.

Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.

Schedule

Four days a week, 10-hour shifts (12:00 PM – 9:30 PM), every other Saturday.
