Hospice Field Admissions RN - Full-Time

Responsible for conducting initial physical and psychosocial assessments of potential hospice patients in various settings to develop personalized care plans, goals, and recommendations based on individual needs. Admits eligible patients into the hospice program according to established standards.

Requirements

• Accredited nursing diploma or degree (BSN preferred).
• Current NYS RN license.
• Valid NY driver’s license.

Qualifications

• 2+ years of experience in med/surg, community health, or oncology nursing (preferred).
