Hospice Field Admissions RN - Full-Time

About the role

Conduct initial physical and psychosocial assessments of potential hospice patients in various settings such as private homes, hospitals, or nursing facilities. Develop individualized care plans, set goals, and make recommendations based on patient needs, ensuring adherence to standards established by the NHPCO. Admit eligible patients to the hospice program and provide compassionate, patient-centered care.

Requirements

• Associate’s/Diploma in Nursing; BSN preferred
• 2+ years’ experience in med/surg, community health, or oncology nursing preferred
• NYS RN license
• NYS Driver’s license
