About the Role

Patient-centered Nursing Care and Case Management of Home Care and Hospice patients in the home setting.

Schedule

PRN / Variable Hours

Pay

$42.66 per hour + differentials based on experience, shift, and PRN status.

Responsibilities

  • Administer skilled nursing care and case management in the home to Home Care and Hospice patients as authorized by the physician.
  • Develop, implement, and evaluate the nursing care plan for each patient.
  • Observe patients for signs and symptoms of complications and report to the physician.
  • Supervise home health aides and LPNs assigned to patients.
  • Promote health and well-being of patients through teaching, supervising, counseling, and appropriate preventative and treatment measures.
  • Utilize community resources and collaborate with other healthcare professionals.
  • Take rotating "call" on weekends, holidays, and evenings to respond to patient needs.
  • Comply with Livingston HealthCare Policies and Procedures.
  • Provide evidence-based care, education, and support for hospice patients, families, and caregivers.
  • Document care responsibly and accurately, including competency in electronic medical records.
  • Track and record mileage on appropriate forms.
  • Actively participate in Quality Improvement and accept accountability for own actions.
  • Complete annual competencies within the assigned time frame.
  • Attend and participate in Hospice Interdisciplinary, Quality Improvement, department staff, in-services, and Home Care team meetings.
  • Maintain the agency’s image of quality care by providing excellent, patient-centered nursing care.
  • Attend the first available Hospice Volunteer training class if not already trained.
  • Give talks to community groups as requested in collaboration with the Home Care/Hospice Manager.
  • Assist in annual Hospice volunteer training as requested.

Requirements

  • Current Montana RN license.
  • BSN, AD, or Diploma in Nursing.
  • Good assessment skills.
  • Ability to function independently.
  • Strong communication skills.
  • Ability to work in a fast-paced environment with multiple interruptions.
  • Own vehicle for visits and a current Motor Vehicle License.
  • Current BLS certification.

Preferred Qualifications

  • At least one year of recent nursing experience, preferably in community health and/or care of terminally ill patients.
