Responsibilities

• Providing skilled nursing services to patients in their homes under the direction of a physician.
• Assess, plan, implement, document, and evaluate patient outcomes.
• Manage a caseload of patients experiencing a wide range of health conditions.
• Provide pertinent patient education, treatment, and emotional support to help individuals recover from injuries and illnesses and/or manage chronic disease processes.
• Demonstrate excellent assessment skills and technical proficiency.
• Anticipate and respond to changes in clinical status of patients.
