About the role

The PRN Registered Nurse manages and coordinates the care of assigned clients in their homes by providing comprehensive skilled nursing services and referrals for therapeutic services as needed, in order to achieve maximum rehabilitation and recovery for the Agency's clients.

Responsibilities

• Complete initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes; assessments may include OASIS items at appropriate time points.
• Accept client admissions as assigned by the Agency and handle weekend on‑call assignments.
• Provide comprehensive nursing care through utilization of the home assessment, nursing diagnoses, and physician‑approved care plans.
• Perform treatments requiring specialized skills as ordered by the physician.
• Observe signs and symptoms and report them to the VP of Clinical Services, Director of Professional Services, and/or the physician as appropriate.
• Note changes in physical or emotional status of clients.
• Assess nutritional needs and diet of clients as ordered by the physician.
• Educate patients and families on medication and procedures.

Qualifications

• Current Registered Nurse license.
• Graduate from an approved school of nursing.
• Minimum of two years of experience in an acute setting requiring heart, lung, and physical assessments, or demonstrated ability/education in the same areas.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Ability to function with minimal supervision.
• Ability to manage multiple projects and changing priorities.
• Strong critical thinking skills.
