As an RN on the Home Care Team, you will provide services to those living primarily in Cleveland – east, west and the downtown district. Further coverage will expand as far west as Vermillion. In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort. Cleveland Clinic Home Care Services is a full service home health agency dedicated to continue the World-Class Cleveland Clinic care at home.
