RN Hospice - PRN

We are seeking a Registered Nurse Case Manager to provide PRN home visits for hospice care. In this role, you will assume full nursing responsibility for implementing and evaluating the Plan of Care for each patient, ensuring holistic support that encompasses physical, psychosocial, emotional, and spiritual needs. You will conduct initial and ongoing assessments, develop care plans collaboratively, document patient responses, and coordinate with families, physicians, and interdisciplinary team members to deliver quality hospice care. The position requires the ability to adapt to diverse home environments and provide compassionate support to patients and families facing end-of-life situations. Qualifications include a current RN license in the state of practice, a valid driver's license, and reliable transportation. Preferred experience includes a minimum of one year of clinical practice and CPR certification or the ability to obtain it within 90 days.
