Role Overview
The Per Diem Registered Nurse Case Manager is responsible for delivering comprehensive hospice care, evaluating patient needs, and developing individualized care plans.
Key Duties
- Conduct initial and ongoing patient assessments
- Coordinate and document the Plan of Care
- Collaborate with patients, families, physicians, and interdisciplinary team members
- Ensure adherence to policies to maintain quality care
Qualifications & Skills
- Unrestricted RN license
- At least one year of clinical experience
- Valid driver's license and reliable transportation
- CPR certification (must be capable of completing)
Pay
$35.45 to $53.15 per hour
