About the Role

The Registered Nurse Case Manager delivers comprehensive hospice care by developing and implementing personalized plans of care, evaluating patient needs, and collaborating with patients, families, and healthcare teams.

This PRN position involves home visits within a designated area, emphasizing compassionate, patient-centered care.

Responsibilities

• Conduct initial evaluations and reassess patient conditions regularly.
• Develop and implement personalized plans of care.
• Document care and patient responses.
• Provide patient education on self-care techniques.
• Ensure quality care in diverse home environments.
• Coordinate care and collaborate with healthcare teams, patients, and families.
• Perform patient assessments and care coordination.

Requirements

• Current RN licensure.
• Valid driver’s license and dependable transportation.
• CPR certification (preferred).
• Clinical experience (preferred).
