About the role

As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and make necessary changes to increase productivity and quality of care delivered. Adhere to and support the policies, procedures, goals, and objectives of hospice to assure quality patient care.

Responsibilities

• Make the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
• Identify the patient's/family's physical, psychosocial, and environmental needs and re-assess as needed, no less than every fifteen (15) days
• Initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
• Document problems, appropriate goals, interventions, and patient/family response to hospice care
• Collaborate with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

Requirements

• Current unrestricted RN licensure in the state of practice
• 1+ years of clinical experience
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
• State Specific Requirement: LA requires a minimum two years full-time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.

Preferred Qualifications

• Current CPR Certification or ability to complete within 90 days of hire

Pay

The hourly pay for this role will range from $35.45 to $53.15 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, and certifications.
