Role Overview

Seeking a Registered Nurse with hospice experience to join the hospice team in St. Louis. This PRN position requires day and night shifts, including weekends, and involves supporting triage needs.

Key Duties

• Evaluate patients, develop individualized care plans, and collaborate with the healthcare team to promote patient-centered, culturally sensitive care.
• Assess patient needs, communicate changes, and ensure evidence-based practice for quality outcomes.
• Assist with assessing patient preferences, barriers, and needs.
• Develop and implement care plans with team and family input.
• Evaluate patient condition changes and update care plans.
• Provide culturally sensitive, compassionate care and advocate for patients.
• Collaborate with interdisciplinary team to ensure continuity of care.
