Hospice RN - PRN
Job Summary
Interim HealthCare® Hospice is seeking a compassionate PRN Hospice RN to join their team. This role involves providing home-based care to hospice patients after-hours, weekdays, weekends, and as needs arise. The ideal candidate will enable patients to spend quality time with loved ones and participate in the interdisciplinary team that carries out the patient's plan of care.
Responsibilities
- Provide home-based care to hospice patients after-hours, weekdays, weekends, and as needs arise
- Enable patients to spend quality time with loved ones and participating
- Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
- Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
- Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
- Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
- Take a holistic approach to care that engages the patient’s mind, body, spirit and family
Requirements
- Registered Nurse (RN) license
- CPR certification
- Valid car insurance
- Prior Hospice experience
- 1+ years
- Strong critical thinking and problem-solving skills
- Excellent communication and interpersonal skills
- Proficiency in medical terminology and understanding of Hospice nursing care
- Ability to work independently and manage time effectively
- Triage any emergent needs and collaborate with physicians to provide comfort and quality care to each patient
- Communicate with medical professionals, the hospice team and others
- Maintain accurate, up-to-date records
- Provide services in accordance with agency policies and regulations that define scope of practice
