About the role
As a RN Home Health in Chattanooga, TN, you will be responsible for providing patient care services in the home setting.
Responsibilities
- Provide comprehensive nursing care to patients in their homes
- Document patient care activities accurately and timely
- Collaborate with interdisciplinary team members to develop and implement care plans
- Ensure compliance with all regulatory requirements and standards
- On-call required if requested, on-call needs are subject to change based on staff and branch needs
Qualifications
- HCHB, Oasis, and HH experience required
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- Current state license as a Registered Nurse - Required
- CHPN Certification - Preferred
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2+ years Home health experience - Required
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelor's of Science In Nursing - Preferred
