Full-time position located in Macon, GA with a flexible Monday-Friday schedule, 8-hour days, and occasional on-call rotation after 5 PM, weekends, and holidays (2 holidays per year). Sign-on bonus up to $7,500 for qualified applicants.

Responsibilities

  • Manage an assigned patient caseload, ensuring timely care and documentation as prescribed in the established plan of care and per MD orders
  • Collect and analyze data from patient assessments, telephonic contacts, and EMR information to determine healthcare needs and available resources
  • Identify high-risk patients for re-hospitalization, falls, or high resource utilization; coordinate additional interventions and case conferencing
  • Prioritize multi-issue situations based on patient health criticality, safety, and service impact
  • Identify and address barriers to resolution (e.g., language, literacy, psychological) with appropriate resources or leadership
  • Develop and oversee a plan of care, communicating effectively with involved parties to support quality patient outcomes and satisfaction
  • Coordinate a multidisciplinary clinical team, supervising LPNs and/or HHAs to promote health and safe outcomes
  • Perform ordered treatments, therapies, and procedures in accordance with State Nurse Practice Act, Home Health Conditions of Participation, JCAHO, and agency policies
  • Evaluate progress toward patient-centered goals

Requirements

  • Graduate from an accredited School of Nursing; BSN preferred
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state of residence/work; if declaring a National License Compact (NLC) state as primary residence, meet home state licensure requirements
  • BLS certification required
  • Additional education, training, certifications, or experience may be required by the department

Physical Requirements

  • Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling
  • Must be able to lift and support up to 35 pounds in handling patients, medical equipment, and supplies
  • Must speak English clearly and understandably
  • Intact senses of sight, hearing, smell, and touch; finger dexterity
  • Critical thinking and ability to concentrate
  • Must respond quickly to changes in patient conditions
  • Physical Abilities Testing required

Pay

Pay range: $35.50 - $53.25 per hour.
