RN Home Care Supervisor - Part Time with Benefits 9am-2pm

About the role

This is a part-time position with a consistent schedule.

Schedule

9:00 AM – 2:00 PM, Monday – Friday (25 hours per week).

Responsibilities

• Provide assessment of newly referred patients and assess health needs in home care setting.
• Work with patient to create a patient-centered plan of care.
• Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient, physician, and family.
• Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
• Provide supervision and training of PCA/HHA staff.

Requirements

• NYS Licensed Registered Professional Nurse.
• Minimum of Associate's Degree required.
• One year of nursing experience required.
• Home care experience a plus.
