About the role
As a Registered Nurse at HCA Healthcare, you’ll have all the staffing support, technology, and resources you need to deliver safe, high-quality care—so you can focus on what you do best. Responsibilities
- Performs all transfer center functions, including centralized patient placement, in a timely, customer-centered professional manner using a computer database consisting of facility service profiles, approved clinical protocols, established patient care protocols, and healthcare information.
- Coordinates transfer of emergent, urgent, and elective patients to a final destination facility from all potential referral sources (hospitals, clinics, post-acute centers, physician offices) via a single call to initiate acceptance and expedite decision-making.
- Facilitates patient throughput to a final destination facility with an ongoing focus on quality and efficiency, providing follow-up to all participants of the transfer.
- Promotes teamwork by demonstrating good team member behaviors, identifying and assisting team members with heavier workloads, and serving as an example of appropriate staff behavior.
- Ensures timely bed placement, appropriate bed assignment, and provides accurate patient information to assist with developing an initial plan of care.
- Ensures accurate and concurrent data collection and maintenance in both patient record and center workflow systems.
- Proficiently navigates and documents in all technology platforms and other call/office-oriented systems as needed.
- Processes patient acuity utilizing critical thinking skills, clinical decision-making, and acquired knowledge to assess situations and facilitate patient flow through the continuum of care.
- Triages and prioritizes patient information, communicating an accurate clinical presentation to accepting physicians, nurses, and hospital administrators.
- Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients of all ages and clinical service needs.
- Serves as a consultant and mediator for physicians, NPs, PAs, nurses, and ancillary staff.
- Communicates professionally with referring and receiving facilities regarding all patient transfers, bed placements, and coordinates patient movement.
- Uses problem-solving skills to determine the best course of action for patient clinical outcomes, considering physicians’ preferences and hospital needs.
- Tracks and trends barriers to patient intake flow; makes recommendations and develops action plans to improve processes and systems.
- Participates in daily bed management meetings (possibly virtual) and effectively communicates resources and issues to appropriate facility areas.
- Demonstrates knowledge of regulatory requirements such as EMTALA, HCA Ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes.
- Adheres to established policy, procedure, and standards of care; escalates issues through the established chain of command in a timely manner.
- Recommends process changes to create a more efficient, customer-centered process.
- Attends staff meetings, completes annual educational requirements, and other specified learning activities on time.
- Utilizes other team members and their experience to ensure the best plan of care for the patient.
- Practices and adheres to the Code of Conduct, philosophy, and Mission and Value Statement.
- Performs other duties as assigned.
Requirements
- Graduate of an accredited nursing program required; Bachelor’s degree in Nursing preferred.
- Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred).
- RN License in the states where the HCA facilities for that Division are located—multi-state license required if the Transfer Center covers more than a single state (may be obtained upon hire).
