About the role

The RN Coordinator plays a central role in patient care, coordinating care across the continuum to achieve optimal outcomes and an exceptional patient experience. Key responsibilities include assessing, planning, and facilitating comprehensive care; directing a team to assist with procedures, monitor patients, and provide appropriate follow-up care; coordinating care through appropriate triage, open access, test verification, and follow-up systems; collaborating with staff, families, residents, and physicians regarding patient needs; developing and monitoring patient education materials; and assisting the Medical Director and Manager to enhance clinical efficiencies.

Responsibilities

  • Demonstrates a high level of clinical competence in providing leadership in coordination of care.
  • Directs a team to assist with procedures, monitors patients, and provides appropriate follow-up care.
  • Collaborates with staff, families, residents, and physicians regarding patient's needs and actively participates in process improvement.
  • Develops and monitors patient education materials.
  • Assists Medical Director and Manager to improve clinical efficiencies by enhancing customer service and implementing employee retention strategies.
  • Provides comprehensive, safe, and effective, evidence-based nursing care to achieve desired health outcomes for the patient.

Requirements

  • Demonstrated clinical competence in coordination of care.
  • Completion of RN program.
  • Three (3) years nursing experience.
  • Registered Nurse: AZ.
  • American Heart Association Basic Life Support - CPR.

Preferred

  • Experience in assigned area.
  • Office or clinic experience.
  • Previous management or supervisory experience.
  • Bachelor's of Nursing.
  • Bilingual.
