About the Role

As a Hospice Registered Nurse Clinical Director in Fall River, MA, you’ll supervise a team providing compassionate end-of-life care while shaping the future of aging in place. This is a full-time, on-site position based at the Fall River office location.

Responsibilities

  • Supervise direct patient and family care team members in collaboration with program site leadership.
  • Handle scheduling, payroll, and other administrative duties.
  • Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions and synchronization.
  • Oversee and support the Interdisciplinary Group (IDG) in developing, implementing, and reviewing the interdisciplinary plan of care for each patient and family.
  • Collaborate with program site leadership to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols.
  • Participate in and promote Back to Bedside efforts to reinforce case management skills, knowledge, and attitude per protocol for all disciplines.
  • Collaborate with site leadership to support staff education efforts.
  • Facilitate IDG meetings and provide ongoing IDG support, including triaging incoming calls, caller support, and quality calls.
  • Participate in or lead quality improvement activities, including data collection and aggregation.
  • Reinforce and support all internal and external communications.

Requirements

  • Associate degree preferred.
  • Certified Hospice Nurse (CHPN) preferred.
  • Three or more years of experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (e.g., geriatrics, pediatrics).
  • Minimum of one year of supervisory experience.
  • Possess and maintain valid CPR certification while employed in a clinical role (preferred).
  • Must be a licensed driver who can travel to all business locations.
  • Meet the regulations and requirements of the state(s) in which the program provides services.

Pay

$95,000 - $115,000 per year (offer based on years of experience).

Schedule

Friday with administrative on-call duties.
