RN Case Manager - Per Diem

About the Role

Responsible for ensuring quality healthcare outcomes through safe, individualized discharge planning and timely utilization management services. The role involves proactive collaboration with patients, families, and healthcare teams to maximize reimbursement and meet patient needs.

Responsibilities

• Establish safe discharge plans tailored to patient needs
• Provide proficient utilization management services
• Collaborate with healthcare team and patients to optimize outcomes
• Ensure documentation and compliance with clinical standards

Requirements

• Graduate of an accredited nursing school
• Minimum three years of clinical experience; two years in Utilization Review, Management, or Case Management preferred
• Knowledge of social and physical factors affecting discharge and community resources
• Current Nevada Registered Nurse license
• Proficiency in computer programs including word processing and data management
• Ability to provide age-appropriate care based on patient demographics
