RN Case Manager - Per Diem

About the role

We are seeking a registered nurse with at least three years of diverse clinical experience, including familiarity with utilization review, management, or case management.

Responsibilities

• Establish safe, individualized discharge plans to optimize healthcare outcomes.
• Provide proficient, timely utilization management services to maximize reimbursement.
• Collaborate proactively with patients, families, and healthcare teams to meet clinical and social needs.

Qualifications

• Graduate of an accredited nursing school.
• Current Nevada RN license.
• Knowledge of social and physical factors affecting discharge and awareness of community resources.
• Proficiency in computer applications such as word processing and data management.
• Ability to deliver age-appropriate care based on patient demographics.
