RN Case Manager - Per Diem

Responsibilities

• Establish safe, individualized discharge plans and provide proficient utilization management services to optimize healthcare outcomes and reimbursement.
• Collaborate proactively with patients, families, and healthcare teams to ensure quality care.

Qualifications

• Graduate of an accredited nursing school.
• Minimum three years of clinical experience; two years in Utilization Review, Management, or Case Management preferred.
• Knowledge of social and physical factors affecting discharge and community resources.
• Current Nevada Registered Nurse license.
• Proficiency in computer programs for data management.
• Ability to provide age-appropriate care to diverse patient populations.
