RN Case Manager - Per Diem

Responsibilities

• Establish safe, individualized discharge plans
• Deliver proficient utilization management services
• Optimize healthcare outcomes and reimbursement
• Collaborate proactively with patients, families, and the healthcare team

Qualifications

• Graduate of an accredited nursing school
• Minimum of three years of clinical experience; two years in Utilization Review, Management, or Case Management preferred
• Knowledge of social and physical factors affecting discharge and familiarity with community resources
• Current Nevada Registered Nurse license
• Computer proficiency in word processing, spreadsheets, and data management
• Ability to provide age-appropriate care for diverse patient populations
