RN Case Manager - Per Diem

About the Role

A registered nurse with at least three years of diverse clinical experience, including two years in utilization review, management, or case management.

Responsibilities

• Establish safe, individualized discharge plans
• Provide proficient, timely utilization management services
• Collaborate proactively with patients, families, and healthcare teams to optimize healthcare outcomes

Requirements

• Graduate of an accredited nursing program
• Current Nevada RN license
• Knowledge of social and physical factors affecting discharge and community resources
• Proficiency in computer applications including word processing and data management
• Ability to provide age-appropriate care for diverse patient populations
