RN Case Manager - Palliative Care - MGH
About the role
The Case Management Department at Massachusetts General Hospital (MGH) is seeking dedicated Case Managers to join our team. This role involves collaborating with the health care team to enhance patient care services along the continuum of care.
Responsibilities
- Evaluate, predict, and facilitate the trajectory of patient care
- Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet individual’s health care needs
- Meet patient’s needs efficiently and expeditiously by continuously improving the patient’s experience
- Help ensure the institutional standards of high-quality patient care
- Reduce cost and ensure reimbursement
Requirements
Registered Nurse [RN - State License] required. Experience in lieu of a degree may be considered.
Qualifications
Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in Microsoft Office applications are required. Knowledge of healthcare regulations and policies is beneficial.
Skills
Excellent organizational and problem-solving skills, attention to detail, and the ability to manage multiple tasks simultaneously are essential.

$40.50 - $100.13/Hourly
