Palliative Case Manager
Job Description
The Palliative Case Manager will collaborate with multidisciplinary team members to provide direct and indirect psychosocial support to palliative care patients through the palliative care services consulting team. This position will also apply knowledge of psycho-social care in support of the hospital quality improvement initiatives.
Job Requirements
Education: Graduate from an accredited school of nursing
Required: Bachelor of Science in Nursing
Preferred: Bachelor of Science in Nursing
Licenses and/or Certifications: Current license to practice professional nursing in the state of Texas
Required: Certified Hospice Palliative Nurse
Preferred: Certified Hospice Palliative Nurse
Work Experience: 3 years of health care experience
Required: 2 years’ experience in hospice, palliative care or related health care setting
Preferred: 2 years’ experience in hospice, palliative care or related health care setting
Qualifications
