Nurse Case Manager - Palliative
About the role
The RN Case Manager utilizes the case management process to provide care coordination services to a group of complex pediatric patients and their families across the continuum of care. Essential functions include assessing patient/family needs, establishing and implementing a plan to proactively facilitate achievement of outcomes, ongoing planning to achieve individualized patient/family outcomes, evaluating patient/family outcomes to continuously update the plan, and documenting the plan and interventions.
Responsibilities
- Utilizes case management process by assessing patient/family needs in hospital, home, or other community location
- Establishes and implements a plan to proactively facilitate achievement of outcomes through communication with patient/family and health care team to coordinate care
- Ongoing planning to achieve individualized patient/family outcomes
- Evaluates patient/family outcomes to continuously update the plan
- Documents plan and intervention
- Participates in Palliative Program development
- Works with the patient/family in collaboration with the payer, community agencies, Hospice agency, schools, etc. to ensure continuity of care and to avoid duplication of services
- Provides professional education to the staff or community
Requirements
Experience: 2+ years pediatric experience required, Case Management experience preferred
Qualifications
- Education: Associate's Degree required, Bachelor's Degree BSN preferred
- Licenses and Certifications: BLS-Basic Life Support Healthcare Provider - American Heart Association/American Red Cross/National Safety Council required, License RN-Registered Nurse (30) - State of Wisconsin required
Skills and Abilities
- Certification as a Case Manager and experience in case management strongly desired
- Requires a working knowledge of families within community systems and application to pediatric case management practice
- Requires a high level of analytical ability to work with complex patients/families medical and social problems across the continuum
- Communication and interpersonal skills which demonstrate effective collaboration with patient, family and health care team members across the continuum
- Computer skills desired including: Microsoft Word, Outlook, Excel
