Responsibilities

  • Comprehensive assessment of patient and family needs
  • Identification of outcomes based on needs assessment
  • Establishing the plan of care
  • Supervision of the progress and modification of the plan of care as needed
  • Maintaining education and competencies to remain proficient in patient care
  • Incorporating evidence-based nursing practice
  • Identifying outcomes that are age and culturally appropriate, realistic, and attainable
  • Collaborating and delegating appropriately to ensure safe and effective care
  • Fulfilling all organizational requirements
  • (Supplemental roles: Charge Nurse, Preceptor)

Requirements

  • Associate's or Vocational degree
  • 2 or more years RN experience
  • Preferred: Bachelor's degree
  • Case Management experience
  • Required: Current BLS certification from the American Heart Association, American Red Cross, or Allina in-house training
  • Nursing Case Management Certification (CMGT-BC) preferred
  • Certified Hospice and Palliative Nursing (CHPN) preferred
  • Valid Driver’s License required within 60 days of employment (MN and WI residents)

Qualifications

  • Licensed Registered Nurse – MN Board of Nursing required if working in state of MN upon hire
  • Licensed Registered Nurse – WI Department of Safety & Professional Services required if working in the state of WI upon hire

Pay

The pay range for this position is $42.92 to $59.38 per hour. Your starting rate will depend on your experience, education, and the union agreement (if applicable).

Schedule

This is a 1.0 FTE (80 hours per two-week pay period) position with 8-hour day shifts and no weekends.
