About the role
The RN Case Manager / Social Worker manages patient care from admission to discharge by assessing, planning, collaborating, and intervening.
Responsibilities
- Conduct initial patient assessments within 48 hours of admission
- Utilize Interqual criteria to determine appropriate levels of care
- Document care and avoidable days
- Collaborate with healthcare team members to ensure safe discharge planning
- Provide patient education on disease management and follow-up care
Requirements
- Current Nevada Registered Nurse or Social Worker license
- At least five years of nursing experience
- Two years in utilization review or case management preferred
- BLS certification required
Skills
