About the role
We are seeking a full-time RN Case Manager or Social Worker to coordinate patient care from admission to discharge.
Responsibilities
- Assess, plan, collaborate, intervene, monitor, and evaluate patient care
- Review care for appropriateness, quality, and efficacy
- Facilitate discharge planning and provide patient and family education
- Document assessments within 48 hours of admission and use Interqual criteria to determine care levels
- Ensure safe discharge and post-acute care services
Qualifications
- Current Nevada RN or Social Worker License
- Minimum five years of nursing experience; two or more years in UR or CM preferred
- Current BLS certification

- Strong assessment, collaboration, and documentation abilities
- Knowledge of care levels and discharge planning
