About the role
Coordinate patient care from admission to discharge as a Full-time RN Case Manager or Social Worker.
Responsibilities
- Assess, plan, collaborate, implement, monitor, and evaluate patient care processes.
- Review care for appropriateness, quality, and efficacy, utilizing Interqual criteria.
- Facilitate discharge planning, including patient and family education on disease management and follow-up care.
- Document assessments within 48 hours of admission and monitor care levels and avoidable days.
- Collaborate with healthcare team members to ensure safe discharge and post-acute care services.
Requirements
- Current Nevada Registered Nurse or Social Worker license.
- Minimum five years of nursing experience and two years in utilization review or case management preferred.
- Discharge planning experience is advantageous.
- Current BLS certification.
