RN Case Manager / Social Worker - Full Time

About the role

Oversee patient care from admission to discharge, ensuring quality and appropriate level of care.

Responsibilities

• Conduct assessments within 48 hours of admission
• Utilize Interqual criteria to determine care appropriateness
• Document avoidable days
• Coordinate discharge planning with the healthcare team
• Provide patient and family education on disease management and follow-up care
• Perform ongoing evaluation and documentation of care processes

Requirements

• Current Nevada RN or Social Worker license
• At least five years of nursing experience
• Two or more years of utilization review or case management experience (preferred)
• BLS certification
